Adult Foster Care

In Indiana MENTOR’s Adult Foster Care program—also known as Host Home—adults and seniors with intellectual and developmental disabilities and other challenges live in private family homes with dedicated caregivers who we call Mentors.

About Us

Indiana MENTOR has been enhancing the quality of life of citizens in the Hoosier state for nearly 30 years.  We provide a wide range of services to adults and adolescents, including Group Home and Day Services.

In all of our programs, we promote learning and growth, and offer a personalized approach which tailors services to each individual’s unique needs.  We believe that anything is possible with the right supports.

We create individualized service plans that are designed by leveraging the unique skills and talents of the adults receiving services and establishing personalized goals. Our continuum of services includes:
